This endpoint is drastically more efficient, and returns a much smaller
response payload, when vulnerability details aren't returned, and
vulnerability details can be looked up more efficiently in the
/vulnerabilities/CVE-XXXX-YYYY endpoint as that endpoint returns the
description once overall rather than once per host.

- Deleted iOS/iPadOS host will continue to report to Fleet as long as
host is in Apple Business Manager (ABM).
- Refetching an offline iOS/iPadOS host will not add new MDM commands to
the queue if previous refetch has not completed yet.

I considered `nano_command_results` but could not think of an efficient
way to use `nano_command_results` to determine if refetch was already
done. The problem is that we're overloading `command_uuid` to include
the `REFETCH` identifier. So we can't simply add an index on `(uuid,
command_uuid, updated_at)`. We need to do a partial text match on
`command_uuid` which then requires a full scan of the matching
`updated_at` timestamps. It feels like a search like this could blow up
for a large `nano_enrollment_queue` and `nano_command_results` table.

`disable_issues` param, plus additional API support for critical
vulnerabilities count
* /api/latest/fleet/labels/:id/hosts now return
`critical_vulnerabilities_count` for premium users.
* /api/latest/fleet/labels/:id/hosts can be sorted by total_issues_count
by specifying `order_key=issues` query parameter.
* /api/latest/hosts/:id and /api/latest/hosts/identifier/:identifier now
return `critical_vulnerabilities_count` for premium users.
* For /api/latest/fleet/hosts, /api/latest/fleet/hosts/report, and
/api/latest/fleet/labels/:id/hosts endpoints, the

Fixed host details page and device details page not showing the latest
software.
- During software ingestion, software titles are now added if needed and
software items have their title_id field populated.
- In addition, after refreshing via UI, the software will be re-fetched
if it has been modified.
Added `exclude_software` query parameter to the
`/api/latest/fleet/hosts/:id` endpoint to exclude software from the
response.

This changes all the places where we previously assumed that certs were
hardcoded when the Fleet server started to query the database instead.
The plan is to loadtest afterwards, but as a first preemptive measure,
this adds a caching layer on top the mysql datastore.
